The final step is to add the text p402_premium to a div class in your blog HTML.
1. Navigate to the Template tab in your Blogger account and click Edit HTML.
2. Scroll down to line 932 and expand using the arrow. On what is now line 935, insert p402_premium so that the line now reads:

3. You should now be able to preview your Blogger posts hosting Google Surveys.
